Caroline M Belfanti

Caroline Belfanti is the Nurse Manager of the Community Palliative Care Team at Calvary Health Care Kogarah, a service recognised as a leader in specialist palliative care. A Clinical Nurse Consultant she brings 29 years’ experience in the community setting working in a culturally and linguistically diverse environment. With a particular interest in models of care, she has through her clinical leadership established shared care models of care, improved clinical practice through a practice development framework, improved collaborative relationships including GP liaison. She has become a regular facilitator invited by Hospis Malaysia to their Annual Palliative Care Nursing Workshops in 2018 and 2019. She is committed to making a difference to individuals and families at the end of life and has been instrumental in helping patients stay at home longer through better patient models and a holistic approach to care. She is also ardent about improving the knowledge in the wider community about palliative care. She is a member of Palliative Care NSW and Palliative Care Nurses Australia.
